If your appointment is between 8:00 AM and 11:00 AM, and you are over the age of 19, please fast 10-12 hours prior to your appointment. You may have water, black coffee or unsweet tea. Drink lots of water prior to your visit. Please arrive 10-15 minutes early to allow us adequate check-in time. Remember to bring your insurance card and driver’s license to the appointment. If you need to cancel or reschedule, please provide a 24-hour notice or you will be charged for a missed appointment. Also, please remember to print and complete the Review of Systems form and bring it with you to your appointment.

Please arrive 5-10 minutes early to fill out any necessary paperwork.

You may leave non-urgent messages with the nurse/medical assistant voice mailbox. These messages are checked throughout the day; please be assured that calls will be reviewed in a timely manner. If you wish for someone to call you back, please leave a number where you can be reached and include your name, date of birth and a detailed message. Please also advise whether we can leave a detailed message on your answering machine if we are unable to reach you. Messages are reviewed and prioritized by the nurses/medical assistants. It could be 2-3 business days for non-urgent calls to be returned.

If you have a co-pay plan, your co-pay is due at the time of service. If you have a Deductible or Coinsurance plan, Carrboro Family Medicine will need a minimum payment of $75.00 at the time of service. This $75.00 payment will go towards your bill/balance. We will still file your claim to the insurance company on your behalf. If you still have a balance after we hear back from your insurance company, we will send you a statement for the remainder wed the practice. Please note, if you do not pay this at the time of service, you will be asked to sign a payment agreement that will be kept on file. We will need to maintain a copy of your insurance card to make sure we have all the correct information to file your claim correctly.

We are happy to complete any medical forms, sports physical forms, school medical or camp forms on your behalf. If your child has a regularly scheduled check-up, we will able to complete the forms at that time. If you need these completed other than at your regularly scheduled appointment, you will be charged according to the following fee schedule. This fee must be paid prior to completion of the form. Immunization and handicap forms are completed at no charge. Please allow 48-72 hours for our staff to complete the forms.

  • School medical form: $10
  • Other adult forms: $15
  • Sports/camp form: $10
  • SSI/ Retirement forms: $20
  • Disability form: $50
  • Medical Records: $25

The lab located inside the Carrboro Family Medicine office is operated in-house. If you receive a bill for lab work, it will be from Carrboro Family Medicine. Our lab takes the same insurances. If you have any questions, please ask our technician or phlebotomist before you have your blood drawn.

Carrboro Family Medicine Center is a network provider for many managed care plans. We also provide billing information and statements for private insurance carriers. Please be aware that patient information sheets and insurance information must be updated yearly. You may download this FORM prior to your first appointment of the year or complete the form at the office. Insurance companies require this annual authorization in order to process your claims. Payment is expected at the time of service. Special arrangements can be made through our office manager for special financial situations. Appointments for sick visits are made on a daily basis. All patients with serious acute illness will be seen. Appointment availability for a physical exam is generally within two to three weeks. Children six months and younger are prioritized for on-time physicals due to immunization requirements. For older children, especially teens requiring sports/camp exams, please plan ahead. Please fill out the patient portion of all physical forms prior to your child's appointment; our providers are required to certify that they have reviewed this information with you. If possible, please complete the immunization information or attach a copy of your child's record. A sports/camp form can be completed based on any full physical done within the past 12 months. Some insurance plans allow one physical per calendar year; others allow one physical per 365 days. If you are unsure of the date of your child's last physical, please inquire to be sure they need an updated exam. Please be very familiar with the specifics of your medical coverage and prescription plan. Please bring your insurance card to every visit.

Patients requesting a copy of their medical records will incur a $25 charge for this service.

Please don’t forget to schedule your Well Child Checks (ages 0-18), Physical Exams (ages 19-64), Annual Wellness Visit (age 65 and over).

Please contact your pharmacy and ask them to e-scribe a refill authorization. Please allow one week for refills to be completed.

If you have a deductible/co-insurance plan, you are required to pay a $75 deposit at the time of service.

Please call (919) 929-1747 if you need to cancel and re-schedule your appointment. Our office requires a notice of 24 hours if you need to cancel your appointment. If you do not meet this requirement, the following fees will be assessed for the missed appointment:

  • Physical Exam: $50
  • 30-minute appointment: $35
  • 15-minute appointment: $25
